The Model of Care (MOC) provides a basic framework under which the Special Needs Plans (SNPs) will meet the needs of each of its enrollees. The MOC is a vital quality improvement tool and an integral component for ensuring that the unique needs of each enrollee are identified by the SNP. Special Needs Plans (SNPs) may only enroll individuals who meet the plan’s specific eligibility criteria and enrollment requirements

The Model of Care (MOC) training is designed to educate Healthcare Providers (HCP) on the structure, processes, and care management systems of Complex Care Management (CCM), Special Needs Plans (SNPs), and Cal MediConnect (CMC), which provide coordinated care to members with special needs described in MOC.
